Requesting records

If you are looking to request personal records please click on the following link and fill out the ROI. Please make sure to fill out all sections with specific date ranges and email it to bbh@mtaonline.net. If you are a patient of Mike, Tracy or Jana and are transferring care please have your new provider fax us a request for records with a specific date range, not, all records. Requesting all records can cause a delay in getting records to a new provider. Records can take up to 15 business days to process and get back.
